In this case you must download and fill the form for Italian municipalities here, and send it to agenzia.coesionesociale@provincia.tn.it and mariagrazia.cobbe@provincia.tn.it.
In you are certified as a family-friendly municipality at national level by the Polish Large Families Association ZDR3+, you must fill in this form. If not, please, contact the Polish Association.
In this case you must contact with the Observatório das Autarquias Familiarmente Responsáveis which is in charge of the process for admission in this country.
